The following items are required to register a new student at Los Alisos Middle School.

  1. Proof of residency (gas, electric, water bill, escrow paper or rental aggreement)
  2. Immunization records
  3. Check Out papers from previous school with school name and address
  4. Copy of student's recent report card
  5. Birth Certificate
  6. Copy of IEP if student is in an RSP/SDC/Speech Special Education Program
  7. If you are residing with a friend or family member and have no proof of residency the following items are required:
    1. You need proof of residency form notarized
    2. Electric, gas, or water bill with the name and address of the person you will be residing with
  8. Legal documents of Custody placement if applicable
  9. If it is necessary for your child to take medication at school, please ask secretaries for a medication form. This form needs to be filled out by Doctor prior to taking medication at school.
