COVID-19: Self-Screening

Self-Screening BEFORE Entering District Property

All employees/visitors must conduct the following self-screening prior to leaving their home.
  1. Have you experienced any of the following symptoms of which first appeared within the last 10 days

Symptoms List*:

  • Fever (at over 100.4 F or 38 C)
  • Feeling Feverish (i.e., Chills/Sweating)
  • Cough
  • Muscle or Body Aches
  • Fatigue
  • Diarrhea
  • Nausea or Vomiting
  • Loss of Taste or Smell
  • Shortness of Breath
  • Difficulty Breathing
  • Headache
  • Congestion or Runny Nose
  • Sore Throat

 

*Please note: This list of symptoms is not complete. Please talk to your medical provider about any of these or other symptoms that are severe or concern you. Some people who have COVID-19 never develop any symptoms. You are permitted to come to school/work within 10 days as long as you test negative for COVID-19 or receive a written doctor’s clearance, your symptoms are improving, and you have been fever-free without fever-reducing medication for at least 24 hours.

  1. Have you tested positive for COVID-19 within the last 10 days?

Please note: You are permitted to return on Day 6 with a negative test on or after Day 5, improved symptoms, and no fever for 24 hours without medication. COVID-19 Rebound: If symptoms return or get worse after ending isolation, follow isolation protocols again. Isolation can end 5 days after your rebound began if you have been fever-free for at least 24 hours and your symptoms are improving. Test is not required to return for those who experience a COVID-19 rebound.

 

Important Information for Close Contacts:

If you have been identified as a close contact of someone who has tested positive for COVID-19 during the infectious period**, you are permitted to come to school/work as long as you remain asymptomatic, monitor for any symptoms that may appear, wear a highly protective mask indoors around others for 10 days after your last exposure, and test negative for COVID-19 between Days 3-5 after exposure. Exception: Those who have recovered from COVID-19 within the last 30 days must follow the actions above but are not required to test between Days 3-5.

 

**Infectious Period: Two days prior to the onset of symptoms for a positive COVID-19 person through the end of their isolation period or, if asymptomatic, two days prior to the date a positive test was taken through the end of their isolation period.

IMPORTANT: Employees/visitors who answer “no” to all of the questions can report to their worksite/appointment. At this time, it is not required to submit your self-screening information to your supervisor. However, it is critical that each employee conducts this process on a daily basis. Employees/visitors who answer “yes” to any question, are instructed to stay home, self-isolate, and read the quarantine instructions found at www.ph.lacounty.gov/covidquarantine as required by the current LA County Department of Public Health Order. Employees should contact their supervisor, or designee, regarding their absence. Supervisors will determine if remote work is available.
